What could cause some managed private files to not be on the server even though Drupal shows the file is being used?

We have a custom Drupal 9 form with a managed file field that allows users to upload a (private) file to the Drupal Commerce order. This works well except that about 5-10% of the time the file is not on the server even though the "Files" report shows the file in the DB and that it is used in 1 place as expected.

The missing files show "permanent" status as expected.

There are no errors in the logs and there does not seem to be anything common with the failed files. The file names are sanitized and re-uploading a failed files works so it does not appear to be anything with the file or filename that is causing the issue.

I'm stuck on this and hope someone could suggest some possible causes of this?
