Ubuntu 22.04 - Accounts (including root) authenticate without a password prompt

Fairly new to Linux and have inherited a few Ubuntu 22.4 servers that permit autologin for all existing accounts. How do I disable autologin?

The following are noted behaviors:

Console login - type root and press enter: No password required

Sudo from a non prived account - type sudo su and press enter: No password required

Grub menu - Drop to root: requires the account password

Password reset - type passwd and press enter: no password/confirmation prompt

So many things wrong. It's hard to pick a place to begin. Passwordless root means the system is assumed to be compromised, and thus not worth repairing. In my organization, those machines would all be wiped and reinstalled today before going home. Except for the fellow who set up passwordless root -- they would likely be sacked on the spot.
